[A Case of Locally Advanced Unresectable Pancreatic Cancer Treated with Chemoradiotherapy Achieving Long-Term Survival].
INTRODUCTION: The prognosis for locally advanced unresectable pancreatic cancer(UR-LA PC)is extremely poor, surgical treatment is difficult, and chemoradiotherapy is recommended. Here, we report a case of a patient with UR-LA PC treated with chemoradiotherapy, resulting in a long-term survival of 3 years and 2 months without recurrence.
PATIENT: A 55-year-old woman with type 1 diabetes mellitus was diagnosed with PC using computed tomography(CT). The tumor was in contact with the portal vein(PV), and severe stenosis of the PV was observed, along with infiltration from the celiac artery(CA) to proper hepatic artery(PHA). Therefore, the patient was diagnosed with UR-LA PC and was administered 6 courses of mFOLFIRINOX. Follow-up CT showed that the tumor had shrunk to 12 mm; however, the narrowing of the PV remained. Radiation therapy(IMRT 48 Gy/15 Fr)was administered, and a complete response was achieved. The patient is currently under treatment-free observation and has survived for 3 years and 2 months.
DISCUSSION: Few cases of long-term survival after chemoradiotherapy for UR-LA PC have been reported. The patient had poorly controlled diabetes and obesity; therefore, the systemic risk of surgery was high, and treatment-free observation was selected.
CONCLUSION: We report the case of a patient with UR-LA PC who was treated with chemoradiotherapy and subsequently survived for 3 years and 2 months.
